Vision and Hearing Screenings
The Ottawa County Department of Public Health will be on campus for their yearly vision and hearing screenings and the schedule is outlined below.
If you have a vision or hearing concern about your child and their grade is not scheduled for testing this year you are welcome to contact the Health Department at 616-396-5266 (English) and 616-393-5780 (Spanish) to schedule an appointment at an upcoming clinic.
If your child is scheduled for testing and you would like to opt-out for any reason please return the attached form to your child's main office by Monday, October 5th.
Students with Digitally Programmable Shunts & other Implanted Medical Devices that may be affected by magnets should not be tested and there is more information attached below. Please make sure to return the opt-out form if this applies to your child so they are aware.
Due to the nature of the equipment used the Health Department has provided opt-in forms for students who have Insulin Pumps and/or Continuous Glucose Monitors.
- If this applies to your child please return either the consent form OR the opt-out form to the main office by Monday, October 5th.
Screening Dates
Tuesday, October 6
Vision screenings for 1st grade, 3rd grade, and 5th grade.
Wednesday, October 7
Kindergarten, 2nd grade, and 4th grade.
Monday, October 26
Vision screenings for 7th grade and 9th grade.
Wednesday, October 28
Vision and hearing RE-screens for those that missed, failed, or forgot their glasses for the Kindergarten - 5th grade dates.
